Title I Parental Involvement Survey

We value your thoughts and opinions! We ask you to complete surveys following our activities and events so we can have your feedback. If you did not have the opportunity to complete the survey at school, please provide us with your thoughts about our activities and programs. You may print the survey and return/email it to the school if you did not complete one at the event. Surveys will only be available for 2 weeks following an event
